The following information and documentation is necessary when you register your child for school:

A student physical health examination (HRS-H3040) performed within 12 months prior to initial entry into a Florida school; including proof of the TB skin test (with results).

A Florida certificate of immunization (HRS form 680), with all immunizations up to date, including proof of Hepatitis B series. All students must have a second MMR immunization. If the first MMR immunization was given before the child's first birthday, a third MMR is required.

Original Birth Certificate Child's Social Security card (optional) Proof of address- your most recent FPL bill or the deed to the property. Emergency Contact Card. Please provide all necessary information.

If your child is not from a Miami-Dade County Public School, please have a copy of the last report card (for grade level verification).

ALL OF THE ABOVE PAPERWORK MUST BE COMPLETED BEFORE THE CHILD CAN BE REGISTERED. INCOMPLETE PAPERWORK WILL NOT BE HELD AT THE SCHOOL WHILE IT IS BEING COMPLETED. PLEASE BE SURE YOU HAVE EVERYTHING YOU NEED BEFORE YOU COME FOR REGISTRATION.
